LPA Certification

Lasting Power of Attorney (LPA) lets you appoint a person you trust to make decisions and act on your behalf if you lose the ability to make these decisions yourself in the future.

Have a discussion with a trusted person and share your intention to appoint him/ her as your Donee. Log into the website of the Office of Public Guardian (OPGO) to complete making your LPA.

LPA Activation

If a person who has previously made an LPA subsequently loses mental capacity, the appointed donee may need medical certification of the loss of capacity before exercising certain powers under the LPA.

Our clinic provides mental capacity assessment for LPA activation, including a clinical assessment, completion of the relevant form and a supporting medical report if required.

The assessment focuses on whether the person is able to understand, retain, weigh relevant information and communicate decisions in relation to the matters covered by the LPA.

Family members or donees are encouraged to bring the patient’s LPA details/reference number and relevant medical records to the appointment.
